Almost every question in this series has one product behind it: one tablet, one label, one answer. "Dewormer" is not like that. It is a job description printed on a box, and at least four different drugs are sold to do that job — with documented safety margins that differ by more than an order of magnitude, labels that disagree with each other about pregnancy, and one active ingredient that quietly turns into another one inside your dog.
Search the question at 11pm and every result tells you the same thing: dewormers have a wide safety margin, a double dose is unlikely to be a problem. That is true far more often than not. What none of those pages ask is the question that actually determines the answer — which drug is in the tube?

Can I give my dog two doses of dewormer?
Not on your own initiative — but before you panic, find the packaging, because there is a real chance the second dose was the treatment working as designed. Unlike a monthly flea preventive or a once-daily tablet, several dewormers are labelled to be repeated within days, and the schedules differ enough that no single answer covers them.
The Merck Veterinary Manual sets the general expectation honestly: "Most anthelmintics have wide safety margins (ie, the dosage that can be given to an animal before adverse effects are induced is much higher than the recommended dosage)."⁵ For the benzimidazole family it even explains why — "the wide safety margin of benzimidazole anthelmintics is due to their greater selective affinity for parasitic beta tubulin than for mammalian beta tubulin."⁵ The drug is built to bind a structure your dog does not have much of.
But "most" is doing work in that sentence, and Merck immediately shows the contrast: for levamisole, "the safety index (SI... ) is not as wide... (SI = 4–6)."⁵ A four-fold margin and a hundred-and-thirty-eight-fold one are both, in Merck's sense, anthelmintics — which is precisely why "dewormers are safe" is too coarse a statement to act on. The useful first move is not reassurance; it is reading the ingredient line.

Which dewormer is actually in your dog?
One of four drugs, most likely, and the box will tell you in a line most people never read. Each targets a different set of parasites, which is why the products are not interchangeable and why owners so often end up giving two of them.
Fenbendazole is the granule you sprinkle on food — Safe-Guard Canine and Panacur C. Its label lists tapeworms (Taenia pisiformis), roundworms, hookworms and whipworms, and it is dosed at 50 mg/kg (22.7 mg/lb) daily, repeated for three days.¹ Pyrantel pamoate is the liquid wormer for puppies — Nemex-2 and its many store-brand equivalents, at 4.54 mg of pyrantel base per mL, one teaspoon per 10 lb.² VCA describes it as treating "intestinal roundworm, hookworm, and stomach worm parasites," acting "within one to two hours."⁷ Note the gap: no tapeworms.
Praziquantel is what fills that gap, and the difference is narrower than "tapeworms or not" suggests. The fenbendazole label covers exactly one tapeworm, Taenia pisiformis.¹ The pyrantel label covers none.² The praziquantel-containing tablets add Dipylidium caninum — the flea tapeworm, the one most dogs actually get — plus Echinococcus granulosus and control of E. multilocularis.³ That is why "it says tapeworms on the box" is not the same claim on two different boxes. And febantel, the third active in those tablets, is the one worth knowing about, because it is the reason two dissimilar boxes can hold the same drug.
There is a fifth thing owners call a dewormer, and it belongs in a different conversation: the monthly broad-spectrum preventive, whose worm cover rides along with a macrocyclic lactone such as ivermectin or milbemycin. Those products are dosed monthly, not in courses, and they carry a breed consideration the intestinal wormers do not — in its discussion of dogs susceptible to ivermectin toxicity, Merck records that "a defect in the gene for P-glycoprotein has been identified in these dogs and also in some Australian Shepherds, English Shepherds, and Shetland Sheepdogs," adding that "genetic testing is now available to identify individual dogs susceptible to ivermectin toxicity."⁵ If the box you doubled was a monthly chew rather than a worming course, that is a different incident with a different answer — start with the heartworm preventive guide instead, and say the breed on the phone.
That dashed arrow is the practical point of this whole article. Febantel, as the 2025 case report below puts it, "is metabolized to the benzimidazoles fenbendazole and oxfendazole."⁴ A dog who gets a Safe-Guard course from the vet and a three-way tablet from the pet shop has had the same benzimidazole twice, and nothing on either carton says so.
How much room does the label actually leave?
More than most owners expect, and the numbers are printed on the labels rather than left to reassurance. This is the part the forum answers get broadly right and never source.
Pyrantel is the standout. Nemex-2's label reports that "the acute oral LD50 of pyrantel pamoate administered in gelatin capsules to female and male dogs is greater than 314 mg base per lb of body weight, which indicates a therapeutic index in excess of 138 times the recommended dosage."² The chronic data is in the same direction: "no significant morphological abnormalities could be attributed to Nemex-2 when administered to dogs at daily dose rates up to 94 mg base per lb of body weight (40x) for periods of 19, 30, and 90 days," in studies that "included nursing pups, weaned pups, adults, pregnant bitches, and males at stud."² Against that, one extra teaspoon is a rounding error.

These are different products with different endpoints — a vomiting rate, a tolerance study and an LD50-derived index — plotted on one axis for scale only, not as a single comparable safety curve. The 138× figure is an index calculated from an acute lethal dose, not a dose anyone has tested as safe.
Fenbendazole sits in the same comfortable territory at the ordinary dose: its label records that "in U.S. clinical studies 3 of 240 dogs (about 1% of the treated dogs) had vomiting associated with the use of the product."¹ That is the baseline at the correct dose — worth remembering when a dog vomits after a doubled one, because it does not automatically mean the extra dose caused it.
Where does that reassurance stop?
At the combination tablets, and specifically at febantel — where there is now a published canine case that the blanket "wide safety margin" line does not cover. It is worth reading carefully, because it is easy to over-read.
In 2025, Frontiers in Veterinary Science published what the authors call "the first report of suspected bone marrow toxicity secondary to febantel administration." A 6-year-old, 35.6 kg neutered male Labrador Retriever was prescribed a three-way tablet for presumptive giardiasis and given the proper dosage for his weight — 1.5 tablets delivering 28.6 mg/kg of febantel, "orally once daily for 5 days." Fifteen days after the course, he presented for lethargy and fever with severe leukopenia (white cell count 0.3 k/µL against a reference interval of 5.6–15.1) and thrombocytopenia. "Pancytopenia was noted, and bone marrow sampling was consistent with hypoplasia suspected to be due to the febantel administration as no other toxic insults were identified." He "made a full recovery in approximately 2 weeks after stopping the febantel."⁴
Three things about that case matter, and the third is the one to hold onto. First, it was a correct dose — which is precisely why "the margin is wide" is not the same statement as "there is no ceiling." Second, the mechanism is not mysterious: the authors note that "fenbendazole is another benzimidazole anthelminthic that has been shown to cause bone marrow hypoplasia and bone marrow necrosis in various species," and febantel becomes fenbendazole.⁴ Third — the honest scoping — it is a single case report, described as suspected, following five consecutive days of dosing, and the dog recovered fully. It is not evidence that one accidental extra tablet causes pancytopenia, and nobody should read it that way. What it justifies is treating a doubled febantel tablet as a same-day phone call rather than a shrug.
The labels draw a second line here. The three-way tablet's own tolerance testing found vomiting and non-formed stools at five times the labelled dose for three days, with elevated liver enzymes in some dogs that resolved within seven days.³ And its contraindication is the opposite of the granule's: "DO NOT USE IN PREGNANT ANIMALS. Dogs treated with elevated levels... of the combination of febantel and praziquantel in early pregnancy demonstrated an increased incidence of abortion and fetal abnormalities."³ Safe-Guard, meanwhile, "is safe for use in puppies 6 weeks or older and adult dogs, including pregnant bitches."¹ One word on the front, opposite instructions inside.
Was two doses actually the instruction?
Very often, yes — and this is the commonest reason people arrive at this question by mistake. Deworming is one of the few treatments in the cabinet where repeating within days is the design rather than the error.
The Safe-Guard label is explicit that the daily dose is "repeated... for 3 days in a row."¹ Nemex-2's puppy schedule lists treatment "at 2, 3, 4, 6, 8, and 10 weeks of age."² The Companion Animal Parasite Council goes further: "puppies should be given anthelmintics starting at 2 weeks of age and repeating every 2 weeks until regular broad-spectrum parasite control begins," and "all pups should be routinely treated with pyrantel pamoate at 2, 4, 6, and 8 weeks of age and then placed on a monthly preventive."⁶
The repetition is not caution — it is biology, and CAPC spells the reason out. Puppies are frequently infected before birth: "transplacental transmission of larvae from the bitch to the fetal pups in utero is an important route of infection for T. canis," and "many of the T. canis larvae ingested by adult dogs become arrested in somatic tissues," where they can be "activated to resume migration late in pregnancy."⁶ A dose clears what is in the gut on the day you give it; larvae still in tissue arrive afterwards. That is why one treatment is not a cure and why the interval is fixed rather than negotiable.
CAPC also explains why the schedule is worth getting right for reasons beyond the dog: "Toxocara spp. are well-documented and important zoonotic disease agents," infection "is most common in children," and larvated eggs "are commonly found in soil collected from playgrounds or parks, and the eggs survive and remain infective for many years."⁶
So before you treat this as an incident, check the label and the discharge notes. A second dose 24 hours after the first may be exactly what your vet asked for.
What should you do in the first hour?
Find the packaging, work out how much of which drug went in, and call — in that order. Have five things ready before you dial:
- The full product name and the active-ingredient line. "Dewormer" is not an answer to the question your vet will ask.
- Whether it was one product twice, or two products. Two different boxes is the more important version of this incident, not the less.
- Your dog's weight, and the weight band on the pack. Combination tablets come in fixed strengths — 22.7, 68.0 and 136.0 mg of praziquantel, with matched pyrantel and febantel — so a tablet sized for a large dog given to a small one is an arithmetic error, not a counting one.³
- How long ago, and whether anything has been eaten since.
- Whether your dog is pregnant, very young, or was unwell beforehand. All three appear as explicit label restrictions.¹³

What should you watch for, and for how long?
The digestive tract for a day or two, and — only for the benzimidazole products, and only if a repeated course was involved — the dog's energy and gums over the following fortnight. The two windows are different lengths, which is the part owners are never told.

One more caution belongs here, and it is about routine rather than accidents. VCA notes that "pyrantel pamoate can be toxic when given regularly over a period of months."⁷ The margin on a single doubled dose is wide; the margin on a habit of extra doses is not the same thing.
How do you stop this happening again?
By writing down the product, not just the dose — because with dewormers the question "did someone already do it?" is the less useful half. What matters is which one they gave.
This category is unusually easy to double, for structural reasons. Treatment is intermittent rather than daily, so nothing in a pill organiser shows a gap. The products arrive from three different places — the vet, the pet shop, the breeder's going-home pack — and none of them knows what the others gave. And the packaging invites the mistake: four drugs, one word on the front, ingredient lines in small print on the back. A household where one person bought a liquid wormer and another gave the tablet from the vet has not been careless; it has been handed a system with no shared record.
The fix is a note made at the moment of dosing — date, product name in full, active ingredient, who gave it — visible to everyone before anyone reaches for a second box.
